How Do You Charge For Road Time Going To Multi-Day Job?

What are some of the ways everyone charges for road time while traveling to out of town project work that may span several days.  

Say you are driving 200 miles to a job site, either the night before or the day of, and are charging a standard agreed to mileage rate of some sort ... however, if you are driving 3+ hours to the site, how do you charge for this time?  Is it at a different rate if the drive time is during normal business hours versus after hours the night before?  How different is this rate from the hourly rate you normally charge for the on site work once there?
