Certificate To Prove CCTV Installation For Insurance

I have a customer who uses USAA for his homeowners insurance and he was told by his agent that he will get a discount if he provides "proof of install" for his CCTV system. My company does not do alarms systems as we mostly do IT Networking and Home A/V. I know you can get a certificate from a monitoring company if you have alarm monitoring but I was wondering does anyone have a template to create one for a CCTV system.

You can call the agent and ask what they deem “proof of install” 

It could be a copy of your invoice marked paid.

I use the same certificate of installation for my monitored alarms as I do for my camera clients. On it I have 2 boxes to check, one for monitoring and one for installation, then a fill in the blank where I can write alarm or surveillance system. Add the clients name, company info, etc and call it good. A quick google search of certificate of installation will pull up a variety of images and templates that should inspire you or allow for a quick modification to produce some nice branded form for your company.
