Reader Locks Up?

I'm pretty stumped on this one, which has been a thorn for me for a while.

I have a R90 (large, long range prox reader) on a parking gate. The reader is about 2 years old. It was replaced after the previous one died during a storm (nearby lighting strike caused a surge I think. I got it on video).
