Any Ideas For Identicard With Reader LED Flashes?

We recently started using IDenticard. After doing a couple projects, one of the team leads noticed that the readers are behaving weirdly. The LED is red during normal operation. When someone presents their badge, the reader turns green for a second, and then starts flashing red-green-red-green, alternating constantly until the door relocks.

There's nothing different about the wiring - we're wiring it just like we used to with Honeywell panels. This is the second project that we noticed this problem on now, and it's using Shlage readers. We used HID readers on the other project. Nobody remembers if this behavior was happening on the first project.
