Do You Offer Maintenance/Service Plans? What Does It Encompass?

Do you offer maintenance/service plans? What does it encompass? Our issue is that we a lot of times get callbacks for small minor items such as password resets, etc... and sometimes they are over a year after the install. While it is billable, the customer sometimes acts as if it shouldn't be. So I am thinking about selling some sort of maintenance/service plan but dont want to re-invent the wheel.
