Consequences Of Ignoring Workplace Safety Measures?

Have you ever encountered a coworker or employee who knowingly disregarded safety protocols? Maybe they refused to wear the necessary personal protective equipment or failed to follow proper installation procedures for a device. What were the consequences of their actions?

In my own experience, I worked for a large corporation that placed a high priority on safety, regularly testing employees on their knowledge of safety protocols such as locking out equipment and wearing appropriate attire. One time, while I had locked out a piece of equipment, a coworker removed a safety device and turned it on, putting me at serious risk of injury. Although this individual was reprimanded and required to retake safety tests, I didn't feel that this punishment was adequate.
