Anti-Wandering Access Options?

One of the toughest problems to fix with access control is elopement prevention or 'anti-wandering' systems that essentially keep people or children from exiting a facility without sounding an alarm or just outright being denied exit. This application is most common to healthcare/ mental health facilities and childcare sites.

How do you normally handle these requests? Do you think a tracking system (free movement) is better than active denial (doors stay locked)?

This looks great on paper, however the cost difference between RFID and a more standard solution is probably pretty steep.
