Access Control Reader Not Working Inside Of Elevator Cab

We have a new install going on in a hospital - controlled access of 4 elevator cabs in a 5 story building. Readers in the elevator Lobby to call the elevator and inside the cab to limit access to specific floors. Installation went fine and we worked with the elevator company to use existing traveling cables for the floors (Basement, 2nd, 3rd and 4th) - however the in-cab card reader keeps giving Misread Errors on a card read - when tied directly to the access panel - they read fine and the correct relays in the elevator fire.

What could be the cause of this? I was thinking grounded wires but that has been checked and wiring seems to be fine. Power perhaps? What else?
